Student Registration

All students registering for school must be accompanied by a parent or guardian.  All forms to be completed for registration will be available on the Tri-Creek web page after August 1, 2011.  You are encouraged to download and complete the forms before coming to the buildings.

 

Indiana law requires all incoming kindergarten students to have a physical examination and proof of immunizations.  All students entering sixth grade must also have a physical examination and proof of a second MMR immunization.

 

Payments may be made into student lunch accounts at registration.

 

Payment of textbook rental and fees is expected at the time of registration. 

Unpaid accounts will be turned over to a collection agency after October 1, 2009.

New to Tri-Creek Schools   If your child will be attending Tri-Creek Schools for the first time, please contact Ronna at 750-1291 to schedule an appointment. You will need to bring the following documents with you: Birth Certificate Immunization Records Proof of Residency Before your child attends school, please download these additional forms to fill out and return to the school at the time of registration . Forms to be completed     If your child is transferring from another school, please provide the office with any school records you may have.   RESIDENCY VERIFICATION Parents and legal guardians of students new to the district will need to meet with the corporation residency official and provide a valid Indiana Driver’s License and either mortgage papers, payment book, or a lease agreement. They must also provide two (2) of the following items: a NIPSCO bill, water bill, pays stub, insurance bill, telephone bill, or bank statement.  All the above items must include parents’ or legal guardians’ name(s) and their current Tri-Creek address   Parents who need to know which Elementary School your child will be attending, please call the Transportation Department at 696-6661.   Students entering Lowell High School should call the Guidance Department (696-7733, ext. 1738) for an appointment beginning August 3.
