Action Plans
For students who suffer from a chronic condition or potentially life-threatening allergy, a parent must sign an Action Plan completed by the student's physician and submit it to the school nurse.

Registration Forms
To be completed upon registration:
- Student Annual Health Information Form - this includes CHIRP consent and consent to treat students in case of emergency.
- Physical Forms (required for Kindergarten and 6th grade students and those participating in school-sponsored athletic programs)
- Kindergarten through Grade 5 students should complete the Tri-Creek Child Health Examination form.

Grade 6 through Grade 12 students should complete the IHSAA Pre-participation Physical Evaluation Form.
